The following is needed prior to setting a date:
Read the booklet in-full
Copy of your child’s certified birth certificate from the county of birth (not hospital certificate)
Fill out the attached Consent form
Each godparent needs to fill out the Godparent Affidavit that is attached (Godparent(s) are affirming to our pastor that what they are filling out and signing is true)
Parents & Godparents need to take an online Baptism class or have taken a class within the last 2 years. The class certificates must be sent along with all other documents.
All documents and information booklet is to the left under "Documents". *Please DO NOT scan or take a picture of the documents with your smartphone unless they are CLEAR and READABLE.
After you have emailed the completed documents to ollvparish@ollv.org, please call the parish office at 702-802-2300 to confirm the documents have been received. You may also make an appointment by calling the parish office, to submit the completed documents in person. Please do not submit documents in person without an appointment.
