BAPTISM information
The following is needed prior to setting a date:
Read the booklet in-full (click on information booklet on the left). The booklet explains all the requirements.
Each godparent needs to fill out the Godparent Affidavit that is attached (Godparent(s) are affirming to our pastor that what they are filling out and signing is true)
A copy of the child's birth certificate must accompany the completed documents.
BAPTISM CLASS - Parents & Godparents need to attend a Baptism class or have taken a class within the last 2 years.Call the parish office for dates and times of upcoming classes. As a courtesy to the priest who is facilitating the class and other attendees, we ask that infants and children do not attend this class.Please call the Parish Office to register at 702-802-2300.
ONLINE Baptism Classes are NO LONGER ACCEPTED. You must attend class in-person at OLLV or another Roman Catholic parish.

After you have emailed the completed documents and a copy of the child's birth certificate to [email protected], please call the parish office at 702-802-2300 to confirm the documents have been received. You may also make an appointment by calling the parish office, to submit the completed documents in person. Please do not submit documents in person without an appointment.
